Across the country, regulatory bodies are cracking down. This is certainly true when talking about government expenditures like Medicaid, and providers need to know how to navigate stricter rules. As state Medicaid programs tighten their oversight through credentialing checks and anti-fraud measures, it’s helpful to have a team by your side who understands these changes. And there’s one key example in Ohio right now: a new ODM 10255 enrollment and revalidation mandate.

Need to walk the fine Medicaid line? Talk to Angela Miller-Leathers at Medical Auditing Solutions. Our team works with offices across the nation to keep them efficient, profitable, and compliant. The Ohio Department of Medicaid (ODM) has recently enacted a mandatory Fraud Penalties Disclaimer requirement; filling out provider fields in the PNM module is not enough anymore. Individual practitioners, facilities, and organizations who accept Ohio Medicaid must sign these forms, and there are real penalties if you don’t.

Make sure you’re doing enough to stay on the right side of Medicaid. Call Angela Miller-Leathers at Medical Auditing Solutions.

Wait, What’s Going On With Ohio Medicaid?

If you haven’t heard, there is a new requirement for both individual practitioners and groups who accept Ohio Medicaid. Beginning just a few days ago on October 6th, 2026, anyone who wishes to enroll as a Medicaid provider in this state needs to sign and submit a document of acknowledgement related to fraud. It’s titled the ODM 10255 – Fraud Penalties Disclaimer, and it’s now mandated by state law.

The goal here is to add stricter acknowledgement requirements and penalties in order to cut down on Medicaid fraud. It needs to be signed by you or someone in your office. Individual practitioners can sign them themselves; groups need to have it signed by the person who’s officially listed as the owner in Ohio’s Provider Network Management information.

Failure To Sign Can Result In Real Penalties

While it might be easy for your office to overlook, it’s essential to sign and submit your ODM 10255 – Fraud Penalties Disclaimer. Ohio’s Department of Medicaid is not playing around here, and they have stated that they will deny the application of a new provider if they do not comply. They can also terminate an existing Medicaid Provider Agreement, so don’t risk your ability to accept Medicaid.

Our office can help you to keep up with the latest changes to state Medicaid requirements for providers and organizations. This form can be uploaded directly to the Agreements page of your file.
